The throughput guide told a client to lease pods and said nothing about
bounding what that lease may grow to, because there was no way to. Adds
the staging recipe (one call per stage, no restart), what the three
$/hr figures now mean and which one to alarm on, and that a `degraded`
status document is stale rather than an idle fleet.
