### 1. ✅ Lost Account Tiers After Server Restart
- Added `account_tiers` table to payment migrations
- Default tier only inserted if table is completely empty

- Added `admin_settings` table to payment migrations
- Payment gateway configs now persist across restarts
### 3. ✅ Analytics Not Recording ALL Requests
- Added analytics for failed rotation requests
- Added analytics for failed autoselect requests
- Added analytics for streaming requests (success & failure)
- Added analytics for authentication failures
- **Result**: 100% of requests now tracked
### 4. ✅ Misleading Migration Log Messages
- Changed "data inserted" to "data checked (existing records preserved)"
- Clarified INSERT OR IGNORE behavior
### 5. ✅ Default Tier Insertion Logic
- Changed from checking `is_default = 1` to checking total record count
- Default tier only inserted if table has 0 records
### 6. ✅ Kiro Token Usage Over-Estimation
- Kiro provider was returning `total_tokens: 0`
- Analytics estimation logic was over-estimating tokens
- **Fix**: Parser now extracts actual usage credits from Kiro API
- Handler uses actual usage data instead of hardcoded 0
- Falls back to 0 if Kiro doesn't provide usage data

## Technical Implementation
### Kiro Token Usage Fix
**Before**:

**After**:

### Analytics Estimation Logic
When providers return `total_tokens: 0`, the estimation logic kicks in:

estimated_prompt_tokens = count_messages_tokens(messages, model_name)
if max_tokens > 0:
estimated_completion = min(max_tokens, estimated_prompt_tokens * 2)
else:
estimated_completion = max(estimated_prompt_tokens, 50)
total_tokens = estimated_prompt_tokens + estimated_completion
```
This was causing high estimates for Kiro because:
- Long conversations have large prompt tokens (40k+)
- Estimation adds completion tokens
- Result: 50k-80k token estimates
**Solution**: Kiro now provides actual usage, so estimation is bypassed.
